Previable PPROM in twin pregnancies: What can we expect?

The aim of this study is to describe the perinatal outcomes twin pregnancies complicated with previable PPROM (PV-PPROM) and expectant management. Retrospective review including women pregnancy before 24+0 weeks ongoing pregnancy, between 2015-2022, in our center. Exclusion criteria were: intrauterine fetal demise (IUFD) one fetus diagnosis, clinical chorioamnionitis at presentation, monoamniotic or triplets higher order pregnancies. Selective termination (ST) entire was considered on parental request fetuses severe prolonged oligohydramnios 23+0 weeks. Perinatal maternal managed expectantly were evaluated. Over period, 45 admitted a diagnosis PV-PPROM Demographic gestational characteristics are shown Table 1. Median (25th;75th percentile) age (GA) 19.3 (16.4-21.6) Seven (15.6%) presented an immediate delivery within first 24 hours since PPROM. Two patients (4.4%) requested ST affected twin. In 36 that opted for management, GA latency 25.4 (22.1-28.6) 37.5 (18-67) days, respectively. Overall survival rate 35/72 (48.6%). 21/36 (58.3%) cases there least 1 survivor 14/36 (38.9%) both survived. There 25/36 (69.4%) who delivered after pregnancy. When periviability achieved, neonatal increased up 80% (35/44). complications related 2. around 50%, similar singleton Once viability reached, increases 80%.
